Purpose of Collection

The personal data that you provided through "MyeHealthPass" website or mobile application (collectively, "MyeHealthPass System") will be collected by Certizen Technology Limited (“Company”) and Smart City Consortium Limited (“Smart City Consortium”, together with Company, collectively,“we”) to verify and used as proof to facilitate your participation in all kinds of activities (collectively, "Promotional Activities") sponsored by third-party organizations (“Organizers”). The personal data you provided in MyeHealthPass System is entirely voluntary.

 

Transfer of Information

Where necessary, under the relevant legislations and/or their subsidiary legislations, for planning and all related purposes, we may provide the information you provided to other authorised departments/organizations/persons, including but not limited to the Department of Health, Hospital Authority and law enforcement officers responsible for epidemiological investigations and contact tracing.

 

Retention of Personal Data

Personal data will not be stored for longer than the time required for the purpose for which it is collected. Users can, on a voluntary basis, directly provide the vaccination record numbers and number of doses taken to participate in all kinds of activities by the Organizers. Based on the information provided, we will assist users to create accounts and passwords in MyeHealthPass System. Users can use the new accounts and passwords to log in MyeHealthPass System to register for the promotional activities by the Organizers. All information collected will only be used for identity verification to register for the lucky draws and promotional activities by the Organizers. The user has complete control over the vaccination record reference number and the number of doses taken stored in the app, and can choose whether or not to present such data to a third party, and can at any time delete such data. The app will not upload these records to any computer systems.

 

In general, personal information stored in the third party will be automatically deleted 90 days after the end of the relevant promotion.

 

Access to Personal Data and Enquiry

In accordance with the provisions contained in the Personal Data (Privacy) Ordinance (Cap. 486), you have the right to request to read and correct the personal data and other related information you provided. Please send an email to info@myehealthpass.com.
